Philip H. Polk, 39, 2420 Pine Ridge Rd., Eau Claire, died Tuesday, November 19, 1996 at his home. He was born November 30, 1956 to Leslie D. and Ruth Anne Polk in Washington D.C. Philip attended Memorial High School in Eau Claire, University of Wisconsin, Madison, and received his Bachelor of Arts Degree from Lawrence University in Appleton, WI. He was active at Immanuel Lutheran Church and University Lutheran Church in Eau Claire.Philip participated in many Human Service activities and spoke often in their groups.\r\nHe is survived by his mother, Ruth Anne Polk of Lawrence, Kansas; 2 brothers, Leslie S.(Jean)Polk, Scottsdale, Arizona, John W. (Sharon) Polk of Lenexa, Kansas; two nephews, William and James, both of Lenexa, Kansas; and a niece, Robin, Scottsdale, Arizona. He was preceded in death by his father, Leslie D. Polk in 1990. \r\nFuneral services will be at 11 A.M. Saturday at Immanuel Lutheran Church, with Pastor David K. Anderson officiating. Burial will be in Forest Hill Cemetery. Friends may call one hour prior to services Saturday at the church.\r\nIn lieu of flowers, the family suggests memorials to the National Mental Health Association.\r\nLenmark-Gomsrud Funeral Home is serving the family.
